IMMUNIZATIONS FOR FALL 2026:
Students entering 7th grade are required to have their Tdap, Meningococcal and all other age appropriate immunizations completed with required documentation submitted to the school nurse prior to starting 7th grade.
Required documentation can include immunization records or exemption affidavits*.
The required documentation can be submitted to the Middle School nurse, Laura Hammack at: [email protected] until the end of the current school year.
After the end of the 2025-2026 school year, these records must be uploaded to Skyward by the parent prior to the start of the 2026-2027 school year.
Individual student schedules will not be released until immunization records are uploaded and up to date.
*Exemption affidavits must have original documents turned in to the school. A link to the affidavit can be found below.
